For a shorter time in the waiting room, please fill out and submit the New Patient Form  below, before your visit.

Our goal is to make you feel welcome, comfortable and heard. We partner with you to care for your ailments. We communicate with your primary care doctor or referring physician through a full use electronic medical record system for timely updates of our visit(s).  In consultation, we obtain a full history and neurological exam that may include standardized written evaluations and questionnaires. Based on our evaluations, we will explain any issues and prescribe treatments (physical therapy, medicines) or further evaluations such as CT scans, MRIs, angiograms, electromyograms (EMGs), sleep studies, evoked potentials, or electroencephalograms(EEGs), if needed.
